My house is listed. Is that the end of it?
Not automatically, but it is a different process and a longer one. Listed building consent applies to internal alterations too, so the pipe route is part of the application rather than a detail.

Does a conservation area mean no?
No. It means the permitted development route closes for anything visible from a road, so a position that is not visible is designed for, or a planning application is made.

Can you do it without a surface run down the wall?
Usually. The pipework can be boxed and painted, run internally where the layout allows, or taken through a void. On a period building it is worth doing properly because a bare surface run is what draws attention.
